Leatherface racconta le origini di una delle figure più terrificanti della storia del cinema horror, il protagonista sadico e crudele della saga cinematografica cult Non aprite quella porta.  Il futuro "Faccia di cuoio", in fuga da un ospedale psichiatrico insieme a tre violenti compagni di ricovero, rapisce una giovane infermeria di nome Lizzy (Vanessa Grasse). La donna viene trascinata nel viaggio infernale che condurrà uno dei quattro verso una serie di indicibili orrori e tragedie. Alle loro calcagna un poliziotto squilibrato in cerca di vendetta. La nascita di Leatherface è più dolorosa di quanto si pensi, con risvolti emotivi angoscianti e complesse implicazioni psicologiche che portano un ragazzino fragile e solitario a compiere da adulto atti di efferata brutalità.
